For example, if a student chose predicted First Class on their application form but gets 2:1 in the end, what would happen?

I was just wondering if there was any instance where an offer was withdrawn just because the candidate got 2:1 instead of a First?
For instance, is there a case where a student who predicted 2:1 did not get their offer withdrawn because they got an average score of 60 but where a student who predicted first got their offer withdrawn because they got 69?
